Regulations of Online Services Rendering

1. GENERAL PROVISIONS

1.1. On the basis of Art.8.1.1 of the Online Services Rendering Law of 18.07.2002,
Med4EU Sp. z o.o. domiciled in Wroclaw, Poland, adopts this Regulations
(hereafter called the “Regulations”) of rendering services online.

1.2. Definitions:
a) Med4EU – the company under the name Med4EU sp. z o.o. domiciled in
Wroclaw, at the address ul. Warsztatowa 11, 53-014 Wroclaw, registered in the
Business Register KRS kept by District Court for Wroclaw Fabryczna in
Wroclaw, Business Department 6 of the National Court Register at the number
0000469197; Med4EU administrates and operates the Website;

b) Med4EU – business name (brand) owned by Med4EU, under which Med4EU runs its business activities connected with organising access to medical treatment
in private medical institutions in the Republic of Poland and with carrying out
refunding of the treatment costs in the Customer’s country of residence;

c) The Website – the web site with the address www.med4eu.co.uk, with all its tabs
and sections, administered and operated by Med4EU;

d) The User – each person who uses the Website, regardless of whether he/she acts
on his/her own behalf or in the name or on behalf of third parties;

e) The Contract – the contract for organising access to treatment in private medical
institutions located in the Republic of Poland and/or carrying out refunding of the
treatment cost in the Customer’s country of residence, concluded in the electronic
form between the User and Med4EU – online via the Website, following the rules
specified in the Regulations;

f) The Customer – the User who concluded the Contract with Med4EU or acts in
order to conclude the contract;

g) The Opinon – Customers’ opinion on the quality of services rendered through the
Website;

h) The Customer Panel – a tool to manage: the Website services and the contact with
Med4EU, available after logging in on the site https://med4eu.co.uk;

i) Registration – the series of Customer actions required to use the Customer Panel
and conclude the Contract.

2. CONDITIONS TO USE THE CUSTOMER PANEL

2.1. Using the Customer Panel, especially concluding the Contract via the Customer
Panel, requires the User / the Customer to read, accept and comply with the contents
of this Regulations as well as the Website and Registration Regulations.

2.2. Technical requirements necessary to use the Customer Panel are specified in the
Website Regulations and require installing on your equipment either Acrobat Reader
or other software for operating PDF files.

3. TYPES AND SCOPE OF SERVICES RENDERED ONLINE

3.1. In the Website, Med4EU allows the User to learn about the possibility to organise
for them an access to medical treatment in private institutions located in the Republic
of Poland and to carry out treatment cost refunding in the Customer’s country of
residence.

3.2. In the Website, Med4EU allows the Customers to conclude the Contract.

3.3. Offering the Users to organise the treatment, Med4EU specifies in the Website,
among others, the following information:

j) legal basis to run cost refunding in the Customer’s country of residence;

k) Price List;

l) General Contract Conditions (OWU) that constitute a sample contract as
regulated by Paragraph 348.1 of the Civil Code.

m) the list of documents required to prepare the application for cost refund.

4. THE WEBSITE USE RULES

4.1. Med4EU provides binding information, advice, recommendation and explanation
solely in the private communication. To this end, the User fills out the contact form
on the Website, giving:

a) name and surname,

b) email address,

c) phone number,

and giving his/her approval to use the personal data in order to execute the application
and for marketing purposes, as well as the approval for online services rendering.

4.2. Start of cooperation with Med4EU and concluding the Contract:

The condition to start the cooperation and conclude the Contract with Med4EU is
registration in the Customer Panel. Registration involves the following steps:

a) giving personal data, marked with a star as necessary, in the Registration Form,

b) accepting the contents of the Regulations and General Contract Conditions
(OWU),

c) giving the consent to process personal data for purposes of the Contract execution;

d) if the User concludes the contract on behalf a child, the Patient’s legal
representative statement is required.

Registration on the Website is considered equivalent to concluding the Contract by
the User. The details how to conclude the Contract on behalf of a minor or person
with limited capacity are specified in point 4.22 of the Regulations.

4.3. The Customer Panel

In the registration process, the Customer sets his/her login and password, which
he/she wants to use to log in to the Customer Panel. After completing the registration
process, the Customer receives an email from Med4EU to confirm his/her [email]
address. After the address is confirmed by the Customer, Med4EU sends another
email confirming the access to the Customer Panel is granted and including a link to
login page.

The customer Panel is used for:

a) the Customer to send scans of documents necessary to prepare and file the cost
refund application as mentioned on the Website;

b) email correspondence between the Customer and Med4EU,

c) Med4EU to send the documentation to the Customer.

4.4. Registration Form
The Registration Form [filled out as] necessary to conclude the Contract online
during the registration, is available after logging in the Customer Panel. The services
specified in the Contract will be rendered under the condition that the Customer
gives all the information required in the Registration Form and continually updates
the information to be accurate and current. Approvals and statements given by the
Customer in the registration process on the Website stay valid during the entire
Contract term.

4.5. Payment forms

Med4EU accepts payments for organising access to medical treatment:

a) via EspagoLink system, by credit or debit card that was approved by the bank for
online transactions, or

b) by traditional bank transfer to the company bank account no. 94 1140 2004 0000
3302 7717 9322 in mBank, which requires sending to Med4EU bank transfer
confirmation by e-mail.

4.6. In the case of payment via EspagoLink system, the payment is operated by the
company called PSP-Polska Spółka z o.o. domiciled in Wroclaw at ul. Lelewela 15,
registered in the Business Register of the National Court Register (KRS) kept by
District Court for Wroclaw Fabryczna in Wroclaw, Business Department 6 of the
National Court Register at the number KRS 0000352235, tax number NIP
8992689516, Statistics Office number REGON 021215833, share capital 50,000.00
PLN – paid-up.

4.7. PSP-Polska Spolka z o.o. does not hold the funds transferred by the Customer, but
only technically enables transfer of the money to Elavon systems, in order to process
and settle the funds for Med4EU. PSP-Polska Spolka z o.o. renders the service in
cooperation with Elavon Financial Services Limited (Spółka z ograniczoną
odpowiedzialnością), Polish Division. Division located at ul. Pulawska 17, 02-515
Warszawa is registered on the National Bank of Poland List of Acquirers that act in
compliance with the Electronic Payment Instruments Law, and hereafter also called
the Acquirer.

4.8. To maintain the safety and confidentiality of the personal and business data, PSP
Spółka z o.o. ascertains and guarantees that it operates in compliance with the PCI
DSS security standard (Payment Card Industry Data Security Standard), as
confirmed by the current certificate available on the company website:
http://www.espago.com.

4.9. After registration on the Website and concluding the Contract, the Customer shall
receive an email, to the email address given at the registration. The email will
contain a personalised link that should be used to make the payment for preparing
application, by credit card on the payment page secured by SSL protocol; the
instruction how to make the payment will be included.

4.10.After opening the [above mentioned] link on Med4EU subdomain, the Customer
makes the payment. To do it the Customer fills in the required data, i.e. name,
surname, card number, CCV number, card expiry date.

4.11.After making the payment correctly to Med4EU bank account and after the payment
verification by the bank (card issuer), PSP Polska forwards to Med4EU payment
confirmation received from the Acquirer, and Customer receives an email
confirmation that the transaction has been completed successfully.

4.12.The Customer payment made via EspagoLink system cannot be cancelled.

4.13.If, due to the Customer data necessary to make the payment being incomplete, the
proper payment transaction is impossible to finalise, then Espago system stops funds
transfer to Med4EU.

4.14.Only the owner, i.e. person whose data are on the card, may use the card on
Med4EU Website.

4.15. Med4EU does not collect and does not store card numbers, only the information on
the card type and four last digits of the card number. Sensitive card data, with the
exception of CVV numbers, are stored by PSP Polska Sp. z o.o. and the Acquirer.

4.16.In the case of payment by traditional bank transfer, the Customer is obliged to
download the transfer confirmation to the Customer Panel. The payment date means
the date of payment as registered by the bank.

4.17.The Customer will receive a VAT invoice from Med4EU for each payment made.
Registering on the Website, the customer accepts receiving VAT invoices without the recipients signature and delivered online to the e-mail address given at the
registration. All invoices will also be available to download from the Customer
Account.

4.18.Any complaints connected with the electronic payment and with the website
operation should be filed either in writing to the mailing address Med4EU sp. z o.o.,
ul. Przasnyska 11A/294, 01-756 Warszawa or online to the e-mail address:
reklamacje@med4eu.com.

4.19.The complaint should include: (1) information on circumstances of the complaint
issue, especially type and date of the fault or malfunction; (2) demand; and (3)
contact details of the person filing the complaint – it will facilitate and speed up the
complaint procedure carried out by Med4EU.

4.20.The complaint will be addressed with no delay, no later than 30 days after its receipt.

4.21.Contract suspension condition
After registration in the Customer Panel and concluding the Contract, according to
General Contract Conditions the Customer is obliged to pay administrative fee at the
amount specified in the Price List. Making the fee payment is the condition to start
services for the Customer by Med4EU within the Contract. After the payment is
made, Med4EU will, with no delay, send by e-mail the confirmation of concluding
the Contract for organising medical treatment. If he payment is not made, the
performance of the Contract is suspended until the payment is made.

4.22.Concluding the Contract for organising medical treatment for a minor or a person
with limited capacity [for legal actions].
In case the Contract for organising medical treatment is concluded on behalf of a
minor or on behalf of a person with limited capacity [for legal action], the
Registration on the Customer Panel requires the Customer’s legal representative to
give all the personal data of both the customer and the legal representative as well as
to file the statement mentioned in Paragraph 5 of the General Contract Conditions. In
such case, Med4EU will download to the Customer Panel additional form to give the
personal details of a minor or of a person with limited legal capacity, which the legal
representative is obliged to fill in. Med4EU recommends that the legal representative
should get to know the information on the treatment of minors and persons with
limited legal capacity in the chosen medical institution.

a) Minors below 13 years of age and other persons without legal capacity
All legal actions connected with concluding the Contract for organising the medical
treatment as well as with the Contract execution are carried out by the legal
representative of the minor Customers below 13 years of age and of other Customer
without legal capacity as if they were done for himself, especially:

–the Contract for organising medical treatment is concluded by the legal
representative of the Customer, who is the party to the contract.
–All statements made by the Customer’s legal representative as if they were his
own

b) Minors above 13 years of age and other persons with limited legal capacity
All legal actions connected with concluding the Contract for organising the medical
treatment as well as with the Contract execution are carried out by the legal
representative of the minor Customers above 13 years of age and of other Customer
with limited legal capacity in the name of and on behalf of the Customer, especially:

–the Contract for organising medical treatment is concluded by the Customer,
and the legal representative of the Customer acts in the name and on behalf of
the Customer.
–All statements are made by the Customer’s legal representative in the name of
and on behalf of the Customer.

4.23.Personal data
The User has the right to see his/her personal data, to change them and to demand its
removal. The User should do it in writing to the mailing address Med4EU sp. z o.o.,
ul. Przasnyska 11A/294, 01-756 Warszawa or online to the e-mail address:
biuro@med4eu.com. In case the personal data is necessary to execute the contract
for organising medical treatment, the demand to remove it is equivalent to contract
termination statement.

5. LIABILITY
5.1. The information, materials or prices presented on the Website are an invitation to
enter the Contract and are not an offer as regulated by Article 66 Paragraph 1 of the
Civil Code

5.2. Med4EU is not liable for the results of events that occur during surfing the Website
that are caused by reasons for which third parties (including telecommunication
companies) are responsible, nor for invalid technical operation of the Website and
interruptions in the Website availability nor for any difficulties caused by force
majeure.

5.3. Med4EU pays attention to risks connected with using the world wide web, such as
the possibility to take over Customer data during its transmission by units not
entitled to it, stealing passwords by third parties, infecting the Customer system by
viruses and damage it may cause, and recommends using the Website with due
caution and care. The Customer should be especially careful to keep the password to
his individual Customer Panel confidential.

5.4. Med4EU is not liable for Customers actions on the Website, neither it is liable for
any consequences of the Customers actions or any actions of third parties that are a
violation of the Regulations. The User is especially liable for:
a) the way the Customer uses the Website and the resulting effects;
b) damage resulting from the Customer action that are a violation of binding law or
provisions of the Regulations.

5.5. According to Article 8.3.2.b of the Online Services Rendering Law of 18.07.2002,
the Customer is forbidden to enter illegal contents to the Website.

5.6. Complaint procedure connected with Med4EU organising access to medical
treatment in private medical institutions located in the Republic of Poland and
executing cost refund in the Customer country of residence is regulated in the
General Contract Conditions.

6. FINAL PROVISIONS

6.1. Med4EU provides the Regulations to the Users free of charge, before concluding the
Contract for organising medical treatment, allowing to obtain, reproduce and record
of the Regulations contents with the information and communication technology
systems used by the Users.

6.2. Med4EU may establish additional regulations, conditions, principles etc. regulating
in detail a specified service or service package offered by Med4EU on the Website.The Regulations provisions are not in any conflict with other contract templates used
by Med4EU, especially General Contract Conditions.

6.3. The changes in the Regulations by Med4EU will be made by publishing new
contents of the Regulations on the Website www.med4eu.co.uk. The new regulations
come into force at the moment of publishing it on the Website.

6.4. Any dispute arising from using the Website will be settled by the court of
appropriate jurisdiction.

6.5. Binding Polish law, especially the Civil Code and the Online Services Rendering
Law of 18.07.2002 apply in the matters not regulated by the Regulations.
