Patients must be ordinarily resident in England and entitled to treatment on the NHS. Permanent residency card is not required.
The treatment is the same as or equivalent to a treatment that would be made available to you on the NHS. Only in exceptional circumstances will NHS England approve other forms of treatment –this will require prior authorisation.
The healthcare provider is based in another EEA (EU, Norway, Lichtenstein, Iceland) country and provides the treatment.
The medical service must be necessary to diagnose or cure disease. The medical need for treatment is assessed by the doctor in Poland, the referral from your GP is not a requirement.
